Sample Instructor E-mail

If a student decides to e-mail a copy of their accommodation letter to their instructor they should include some information in the body of the e-mail to be clear about what is attached and how they plan to discuss this information with the instructor (i.e. office hours, phone meeting, make an appointment).  The student should send a separate e-mail to each of the instructors the student needs to work with.

Sample E-mail Template (for students with exam accommodation needs)

E-mail Subject: Disability Accommodation Letter for <Student name>

Attached please find a copy of my accommodations letter as approved by Disability Services for the current semester.  I would like to set-up a time to discuss this information with you to determine how we will implement these accommodations and to answer any questions you may have about my accommodation needs.
